For Participating Providers

The Mecklenburg County Medical Society and Physicians Reach Out tremendously appreciate the participation of each and every health care provider. Whether a physician, physician assistant, nurse practitioner, hospital, physical therapist or any other provider of services on behalf of Physicians Reach Out, we recognize the vital importance of your contribution to this effort to make access to health care possible for those who so desperately need it.

If you are a physician practice, please download our Physician Office Manual. The information contained in the manual gives you most of what your practice needs, including services authorized, claims filing and patient responsibilities. If you are not with a physician practice, much of the information is still applicable.  

Physicians Reach Out patients may obtain many of their prescription medications from MedAssist of Mecklenburg. MedAssist can only dispense medications that are listed on the MedAssist formulary (click this link to go directly to the MedAssist formulary).

As a participating provider, you may wish to refer your existing patients who may be eligible for PRO. If so, please click here for a referral form.

If you are not a participating provider, and would like to become one, please download the commitment form and fax it to (704) 376-3173. Otherwise, call (704) 376-3688 and Physician's Reach Out will mail you the commitment form.
